Provides information and referrals to support individuals and families affected by Alzheimer's disease or another dementia as soon as possible after diagnosis. Once a referral is received, a customized information package is sent. Follow-up contact from a coordinator continues after three months, six months, one year and further as needed. A bi-monthly newsletter with information such as educational and support group sessions, social programs, and other community programs, is also provided.
